Two different objects
Structured's central object is a day. You put tasks and appointments onto a vertical timeline, see the gaps, and get a sense of shape that a list cannot give you. It is one of the better answers to time blindness available on Apple platforms, and it is polished.
Orbit's central object is a routine. It does not know about your calendar, your appointments, or your afternoon. It knows about a sequence of steps and how to move you through it without losing you between step three and step four.
Comparing them is like comparing a map to a pair of shoes.

Where Structured is genuinely better
Everything to do with seeing the day. The timeline is the feature, and it is very good at what it does.
Devices. iPhone, iPad, Mac and Apple Watch, with sync between them.
Calendar. It sits on top of your existing appointments instead of ignoring them.
Track record. A large, long-standing user base and an enormous number of ratings, which is worth something when you are trusting an app with your day.
Where Orbit is genuinely better
Inside a routine. Structured can remind you that a routine is due. Orbit stays with you through it, showing the current step outside the app so a distraction costs seconds instead of the whole sequence.
Missed days. Orbit's streak freezes absorb them automatically. Nothing resets.
Starting friction. One tap and the routine is running. Nothing to arrange first.
Privacy posture. No account, on-device storage, optional sync through your own iCloud.
How to choose
If you are choosing one app and your problem is "where did the day go", choose Structured.
If your problem is "I know exactly what my morning should be and I still end up leaving the house forty minutes late", choose Orbit.
If you have both problems — many people with ADHD do — the combination works better than either alone, because they are solving different halves of the same bad morning.
